Responsibilities

Position Summary:


Responsible for the medication administration of assigned nursing unit.


Essential Duties:


• Administers medication by oral, rectal, topical, inhalation, eye or ear routes, under the supervision
and direction of a Registered Nurse.


o Follows standard nursing procedure and facility policies for administering medication.
o Maintains medication cart and ensures storage area is clean, neat, and areas are locked if
appropriate.
o Obtains permission from licensed nurse before administering PRN medications.
o Performs the count of countable medications with another medication technician or a
licensed nurse at beginning and end of shift. Does not administer any injectable medications.
o Checks supply of medications for each resident on unit and re-orders meds as necessary from
pharmacy. Does not call in new medications to pharmacy.
o Takes vital signs, following correct procedure, including apical pulse rates.


• Performs the following nursing documentation as assigned by the charge nurse or supervisor.


o Reports any significant changes in resident condition or vital signs to charge nurse or
supervisor promptly.
o May be required to document observations in nursing notes of residents’ records.
o May be required to document treatments and observations on treatment record.
o Documents medications administered on MAR per facility policy and procedures.
o Follows plan of care as directed by RN/LPN. Provides input for evaluation of the plan of
care.


• Partial Admission:


o Takes initial vital signs and measurements and documents them on the vital sign flowsheet.
o Assists with orientation of patient/resident/family to unit, facility and services.


• Plan of Care:


o Delivers patient/resident care promptly and accurately as directed by the RN/LPN and in
accordance with the patient’s/resident’s written plan of care.
o Contributes relevant and appropriate information to RN/LPN for revision of the plan of care,
and interdisciplinary conferences as appropriate.
o Reinforces compliance of RN/LPN teaching and plan of care.


• Performs safe patient care according to policy and procedure


o Obtains and promptly records routine:


? Vital Signs
? Heights and weights as indicated.
? Reports changes in vital signs or measurements promptly to the Supervisor/Charge
Nurse.


o Assists the patient/resident with routine activities of daily living:


? Feeding: verifies diet order with plan of care, feeds or assists with feeding, assists
with fluid intake as directed by RN/LPN or plan of care
? Bathing: verifies activity restrictions with plan of care, bathes or assists with
bathing/shower provides personal hygiene as needed
? Ambulating: verifies activity level with plan of care support patients/residents in
achieving maximum mobility by repositioning, range of motion, and assisting with
ambulation.


o Performs specific ambulation/exercises as directed by RN/LPN. Seeks assistance as needed.
o Provides comfort measures such as PM care and repositioning with proper alignment
o Observes patients/residents to insure safety as directed by the plan of care
o Utilizes proper body mechanics to protect self and patient/resident
o Identifies potential and actual safety hazards and reports to RN/LPN.
o Sets up and checks equipment for proper functioning; reports malfunctioning to RN/LPN.
Labels equipment with reason for malfunction as applicable.


• Skin Care: assists with maintaining skin integrity and reports all pressure areas/sores to RN/LPN.


o Performs routine skin care with bathing, and additional care as directed by RN/LPN

o Reports any redness, drainage and/or swelling promptly to RN/LPN.
o Follows procedures for Universal Precautions at all times.
o Monitors and promptly records patient/resident intake and output in accordance with unit
standards or physician orders including:


? Intake and output
? Fluid restrictions
? Calorie counts


o Reports changes in patient’s/resident’s status (e.g. changes in vital signs, level of
consciousness, weight, mobility status, comfort level, feeding pattern or any unusual
occurrences) to the RN/LPN in a timely manner.
o Answers all call lights promptly and intervenes appropriately.
o Documents care provided on CNA ADL Flowsheet.


• Assists with the following duties as directed:


o May be assigned to transport residents in facility-owned vehicles.


? Ensures that the duties outlined in the “Transportation Aide” job description are
followed when performing this task.


o Prepares patient’s/resident’s room for admission, discharge and transfer.


• Maintains a neat, clean and safe environment


o Keeps patient/resident area clean and free of trash and clutter.
o Keeps linen appropriately stored or disposed.
o Removes equipment and supplies not in use; cleans equipment and returns to appropriate
location for storage.
o Is alert to safety hazards and takes appropriate action.
o Provides safe delivery of patient/resident care.
o Participates in Quality Improvement activities as directed by RN/LPN.
o Seeks ways to improve the individual resident’s quality of life, treats residents with dignity,
and as individuals in order to meet their needs and interests.
o Creates a stimulating, home-like environment where residents experience a sense of
belonging.
o Preparation of between meals snacks, meal set-up and feeding in accordance with established
protocols.
o Responds to resident behavioral problems, observing policies and protocol for appropriate
behavior management.
o Responsible for routine cleaning of equipment, as directed.
o Participates in walking rounds/report and the beginning and end of each shift.
o Maintains residents’ rights and treats resident information with confidentiality.
o Engages resident’s in one-to-one activity as needed. Is alert to needs of individuals not
involved in groups. Is aware of available resources.
o Consults with Activity Director and Department regarding program improvement: new ideas,
adaptations, different techniques, changes in scheduling and programs, etc.


• EDUCATIONAL COMPONENT


o Attends at least (12) twelve role related in-services per year.
o Attends and participates in related in-services, conferences and meetings if applicable.
o Communicates personal and work-related needs clearly to the DON/Charge Nurse.
o Seeks feedback to enhance own performance.
o Manifests desired behavior change in response to guidance.

 

Qualifications

Education and Experience:


• Active Certified Nursing Assistant certification in the state of Maine.
• Active Certified Nursing Assistant/ Medication certification in the state of Maine.
• High school diploma or equivalent preferred.
• Previous experience in Long Term Care resident care helpful.
