Responsibilities

Position Summary:


The Social Worker is responsible for providing social service functions for the rehab unit. The incumbent
shall be responsible for providing case coordination and full range of social work services including
assessment, treatment planning, and therapeutic interventions in an interdisciplinary environment consistent
with the position’s qualification, professional practices and ethical standards. The incumbent shall also
demonstrate accountability for and contribution to program development, quality improvement, problem
solving and productivity enhancement in a flexible interdisciplinary fashion. Assists in performance
improvement activities within the department and participates in CQI activities.


Essential Duties:


• Performs initial assessments on patients to determine appropriate social service needs and initiates
discharge planning needs.
• Develops individualized treatment plans; always documents assessments.
• Meets regularly with patients and families to enhance coping skills, adjustment to disease process, and
strengthen the natural support network.
• Documents Interactions.
• Performs all aspects of patient care in an environment that optimizes patient safety and reduces the
likelihood of medical/health care errors.
• Communicates with physicians regarding social service and complex discharge planning needs of the
patient.
• Maintains current knowledge of resources available within the community.
• Maintains supply of resource materials to be distributed to patients when needed.
• Is able to obtain other resources as needed.
• Communicates with family members and caretakers regarding the needs of the patient and anticipated
plans.
• Collaborates with Case Managers and other members of the healthcare team to provide for
psychosocial assistance.
• Treats patients and families with respect and dignity.
• Identifies and addresses psychosocial needs of patient and family.
• Manages and operates equipment safely and correctly.
• Interacts professionally with patient/family and involves patient/family in the formation of the plan of
care.
• Acts on identified performance improvement issues as appropriate.
• Actively participates in Multi-disciplinary team meetings and other meetings.
• Maintains and respects confidentiality of patient/physician information.
• Completes Health Educational requirements as required.

Qualifications

Education and Experience:


• Licensed or license eligible as a Social Worker
• 3 or more years of previous experience in all aspects of social service.

Knowledge, Skills and Abilities:


• Able to communicate effectively in English, both verbally and in writing.
• Additional languages preferred.
• Basic computer knowledge.
• Discharge planning knowledge/experience helpful.
• Ability to engage patients and team members utilizing the CMH Experience Standards
